Section 48(15) of the Ontario Income Tax Act properly deems a form purporting to be prescribed or authorized by the Provincial Minister (here the Minister of National Revenue by virtue of the collection agreement) to be a form prescribed by order of the Provincial Minister under the Ontario Act unless it has been called into question by the Provincial Minister or a person acting for the Provincial Minister or Her Majesty; waiver form T2029 met those criteria, was not called into question, therefore validly waived the limitation period and the reassessment was timely; the appeal is dismissed.
