Active Navigation Limited, through its financial controller Mrs Andrews, was negligent in failing to ensure the acceptance of increased cover was returned to the insurer and in failing to follow up on the lack of confirmation and premium increase, resulting in loss to Mrs Wade. There was no express or implied contractual obligation to guarantee payment of four times salary irrespective of insurance, and no breach of trust as the required premiums for the cover actually in place were paid.
