No extension of the 12-month period was sought, and no steps were taken to comply with the requirements for permanent registration under section 8 of the Act. The respondent's reliance on incorrect registration numbers and Departmental errors did not establish the existence of a permanent CPA. The Department's subsequent involvement and meetings were misdirected, as the provisional CPA had lapsed and no permanent CPA was ever registered.
