H35 Regulation 65/2007 Registered March 28, 2007 bilingual version (HTML) Definition 1 In this regulation, " Act " means The Health Services Insurance Act . Insured person to provide the following information to the minister 2 For the purpose of clause 98(b) of the Act, an insured person must provide the following information to the minister: (a) the insured person's name, date of birth and mailing address; (b) the date, location and a description of the negligence or other wrongful act or omission that caused the insured person's bodily injuries; (c) the name and mailing address of the person liable for the negligence or other wrongful act or omission, and, if known, that person's insurer and policy number; (d) a description of the bodily injuries suffered by the insured person as a result of the negligence or other wrongful act or omission; (e) details of the insured hospital, medical or other health services provided to the insured person as a result of the bodily injuries; (f) the name of each person who provided the insured hospital, medical or other health services, and if the services provided by that person were provided at a hospital, the name of the hospital; (g) the name and mailing address of the insured person's lawyer.
