Interpretation 2 In these regulations, “Act” means The Public Health Act, 1994. Forms prescribed 3(1) The forms set out in the Appendix are the forms prescribed for the purposes set out in this section. (2) Form 1 is prescribed for the purposes of subsection 29(3) of the Act. (3) Form 2 is prescribed for the purposes of subsection 29(5) of the Act.
