U. z 2013 r. poz. 224 oraz z 2014 r. poz. 1399 i 1698) załącznik nr 1 do rozporządzenia otrzymuje brzmienie określone w załączniku do niniejszego rozporządzenia. Rozporządzenie wchodzi w życie z dniem 1 stycznia 2016 r.
