U. z 2015 r. poz. 1852) § 5 otrzymuje brzmienie: Formularze, o których mowa w § 2, składane w formie dokumentu elektronicznego są opatrywane kwalifikowanym podpisem elektronicznym albo podpisem potwierdzonym profilem zaufanym ePUAP. Rozporządzenie wchodzi w życie po upływie 14 dni od dnia ogłoszenia.
