Załącznik nr 1 - Ocena wskazująca czas powstania zabytku (wzór) Załącznik nr 2 - Wycena zabytku (wzór) Załącznik nr 3 - Potwierdzenie wwozu zabytku na terytorium Rzeczypospolitej Polskiej (wzór) Na podstawie art. 59 ust. 5 ustawy z dnia 23 lipca 2003 r. o ochronie zabytków i opiece nad zabytkami (Dz. U. z 2018 r. poz. 2067 i 2245 oraz z 2019 r. poz. 730) zarządza się, co następuje: Określa się wzory dokumentów: oceny wskazującej czas powstania zabytku, stanowiący załącznik nr 1 do rozporządzenia; wyceny zabytku, stanowiący załącznik nr 2 do rozporządzenia; potwierdzenia wwozu zabytku na terytorium Rzeczypospolitej Polskiej, stanowiący załącznik nr 3 do rozporządzenia. Dokumenty, o których mowa w § 1, ich wydruki i kserokopie mają format A4. Dla wzoru dokumentu: oceny wskazującej czas powstania zabytku ustala się odcień koloru żółtego o symbolu PANTONE 120 U, RGB 255/217/106, CMYK 0/8/66/0; wyceny zabytku ustala się odcień koloru zielonego o symbolu PANTONE 367 U, RGB 143/209/106, CMYK 42/0/74/0.
