§ 3. Załącznik nr 1 - Orzeczenie lekarskie (wzór) Załącznik nr 2 - Orzeczenie psychologiczne (wzór) Na podstawie art. 33a ust. 4 ustawy z dnia 22 sierpnia 1997 r. o ochronie osób i mienia (Dz. U. z 2018 r. poz. 2142 i 2245 oraz z 2019 r. poz. 1495) zarządza się, co następuje: W rozporządzeniu Ministra Zdrowia z dnia 21 grudnia 2015 r. w sprawie badań lekarskich i psychologicznych osób ubiegających się o wpis lub posiadających wpis na listę kwalifikowanych pracowników ochrony fizycznej (Dz. U. poz. 2323) wprowadza się następujące zmiany: załącznik nr 2 do rozporządzenia otrzymuje brzmienie określone w załączniku nr 1 do niniejszego rozporządzenia; załącznik nr 3 do rozporządzenia otrzymuje brzmienie określone w załączniku nr 2 do niniejszego rozporządzenia. Orzeczenia lekarskie i psychologiczne, o których mowa w rozporządzeniu zmienianym w § 1, w brzmieniu nadanym niniejszym rozporządzeniem, mogą być wydawane według wzorów określonych w rozporządzeniu zmienianym w § 1, nie dłużej jednak niż do dnia 31 grudnia 2021 r. Orzeczenia lekarskie i psychologiczne wydane przed dniem wejścia w życie niniejszego rozporządzenia zachowują ważność do czasu upływu terminów w nich wskazanych.
