Dokumenty, o których mowa w § 1 i § 2, mają postać papierowej książeczki, zszywanej nicią przez grzbiet, o formacie 128 mm × 88 mm. Dokumenty stwierdzające uprawnienie do wykonywania zawodu lekarza i lekarza dentysty wydane przed dniem wejścia w życie niniejszego rozporządzenia, zgodnie z wzorami określonymi w przepisach dotychczasowych, zachowują ważność. Rozporządzenie wchodzi w życie z dniem następującym po dniu ogłoszenia.2)Niniejsze rozporządzenie było poprzedzone rozporządzeniem Ministra Zdrowia z dnia 16 czerwca 2003 r. w sprawie wzorów dokumentów: Prawo wykonywania zawodu lekarza, Prawo wykonywania zawodu lekarza stomatologa, Ograniczone prawo wykonywania zawodu lekarza, Ograniczone prawo wykonywania zawodu lekarza stomatologa (Dz.
