U. poz. 430 oraz z 2021 r. poz. 1186) załącznik nr 1 do rozporządzenia otrzymuje brzmienie określone w załączniku do niniejszego rozporządzenia. Do dnia 31 grudnia 2022 r. może być stosowany wzór określony w załączniku nr 1 do rozporządzenia zmienianego w § 1, w brzmieniu obowiązującym przed dniem wejścia w życie niniejszego rozporządzenia. Rozporządzenie wchodzi w życie po upływie 14 dni od dnia ogłoszenia. patrz oryginał
