U. poz. 1656) wprowadza się następujące zmiany: w § 3 pkt 8 otrzymuje brzmienie: uwierzytelnioną przez jednostkę organizacyjną Zakładu lub kierownika ośrodka pomocy społecznej, a w przypadku przekształcenia ośrodka pomocy społecznej w centrum usług społecznych na podstawie przepisów ustawy z dnia 19 lipca 2019 r. o realizowaniu usług społecznych przez centrum usług społecznych (Dz. U. poz. 1818) - dyrektora centrum usług społecznych, jeżeli kierownik ten lub dyrektor składa wniosek o rentę socjalną, kopię dokumentu lub dokumentów potwierdzających okoliczności wymienione w art. 2 pkt 2-4 ustawy z dnia 27 czerwca 2003 r. o rencie socjalnej, zwanej dalej „ustawą”. załącznik do rozporządzenia otrzymuje brzmienie określone w załączniku do niniejszego rozporządzenia.
