W przypadku, o którym mowa w ust. 4, pracownik socjalny wypełnia część VII kwestionariusza wywiadu jedynie w zakresie lit. A poz. 1-6 oraz lit. B. Rozporządzenie wchodzi w życie z dniem następującym po dniu ogłoszenia.
