§1135.4. Ambulance services; fees Any person, partnership, corporation, unincorporated association, or other legal entity currently operating or planning to operate an ambulance service shall pay the following fees to the department, as applicable: (1) An initial licensing fee of one hundred fifty dollars, to be submitted with the initial application for a license.
