Minnesota Statutes § 593.40 - QUALIFICATION QUESTIONNAIRE
A prospective juror who does not return the questionnaire can be ordered by the court to appear and explain the failure, and failing to do so or willfully making a material false statement on the questionnaire is a misdemeanor.
