An application for a medical laboratory license shall be made under oath, by the owner and director of the medical laboratory or public official responsible for the operation of a city or county medical laboratory or institution that contains a medical laboratory, upon forms provided by the board; provided, that the laboratory is in compliance with the regulations promulgated under the authority of this chapter. Each license shall be renewed by making application on forms provided by the department and submission of the fee as set by the board. If the licensee fails or neglects to renew the license by the prescribed time, the license may be restored by paying all registration fees due, plus a late monetary penalty and upon finding there are no outstanding violations attributed to the licensee.
