Occupations Code § 651.260

Sec. 651.260. DUPLICATE LICENSE. (a) A license holder may apply to the commission for a duplicate license: (1) if a license issued under this chapter is lost or destroyed; or (2) as needed to comply with Section 651.261. (b) The license holder must submit the application on a form prescribed by the commission and must submit with the application an affidavit verifying: (1) for an applicant under Subsection (a)(1), the loss or destruction of the license; (2) that the license holder is the person to whom the license was issued; and (3) for an applicant under Subsection (a)(1), any other information concerning the loss or destruction of the license the commission requires. (c) The commission shall issue a duplicate license after receipt of the information and payment of the fee set by the commission for the duplicate license. Acts 1999, 76th Leg., ch. 388, Sec. 1, eff.
