Sec. 522.061. NOTIFICATION OF CONVICTION TO DEPARTMENT OR EMPLOYER. (c) A notification to the department or an employer must be in writing and must contain: (1) the driver's full name; (2) the driver's license or permit number; (3) the date of conviction; (4) the nature of the violation; (5) a notation of whether the violation was committed in a commercial motor vehicle; (6) the location where the offense was committed; and (7) the driver's signature.
