Occupations Code § 351.262
Optometrists and therapeutic optometrists must give patients a prescription, bill, or receipt with the provider’s name and license number, and certain off-site practitioners must also give eyeglass patients a written specification, prices, and a bill with signature, mailing address, and license or certificate number.
