These rules amend the Births and Deaths Registration (General) Rules by replacing Form No. 25 with a new form in the Appendix.

These Rules may b e cited as the Births and Deaths Registration (General) (Amendment) (No .2) Rules, 2004, and shall be read as one with the Births and Deaths Registration (General) Rules, in these Rules referred to as the principal Rules. The First Schedule to the principal Rules is amended by the revocation of Registration General Form No. 25 and the substitution therefor of the form set out in the Appendix of these Rules.
