Although a disciplinary hearing was held on May 27, 2017, the actual termination was based on an alleged hearing on June 22, 2017, which the claimant did not attend and for which no evidence of invitation was provided. The respondent did not clarify or correct the inconsistencies in the termination notice. As such, the termination was procedurally unfair.
