RSA 151:25-a. Nursing Home Facility Statement Required.

Any nursing home licensed under this chapter, which offers medical services shall provide a written statement setting forth the following to prospective clients: I. A specific description of the medical services offered on site. II. The rates charged by the nursing home and what is included in the rates. III. The client's rights in the event of a temporary absence from the nursing home facility, in accordance with RSA 151:25, and the client's rights in the event of a proposed transfer or discharge from the facility, in accordance with RSA 151:26. IV. The circumstances under which a discharge or transfer to another facility may occur, including situations in which the client's needs would exceed what the nursing home could provide, and the responsibility of the nursing home in transitioning the client to another location. V. A statement affirming that the prospective client has received the nursing home disclosure statement in accordance with this section, that the client has read it or it has been read to the client, and that the client understands its contents.
