The coroner found on the balance of probabilities that the nine deaths resulted from multiple injuries and burns caused when the hot-air balloon caught fire during landing, the fire arising from a fuel leak at the upper portion of the forward right hose connected to burner number 193; the deaths were accidental. Systemic failings identified included maintenance and record-keeping deficiencies at the local operator and inadequate risk assessment and information provision by the Hong Kong travel company and industry body, warranting specific recommendations to prevent recurrence.
