On balance of probabilities the coroner found the immediate cause of the fatal fires was a fuel leak at the upper portion of the forward right hose connected to burner 193 which ignited during landing procedures; all nine deaths were accidental; deficiencies in information, risk assessment and supervision by KUONI and its DMC were identified and non‑binding recommendations were made to TIC and KUONI to prevent recurrence.
