On the balance of probabilities the balloon fire resulted from a fuel leak at the upper portion of the forward right hose connected to burner number 193 which ignited and caused multiple injuries and burns leading to death; the nine deaths in this inquest are therefore found to be deaths by accident and the coroner issues targeted recommendations to TIC and KUONI to address deficiencies in risk assessment, information provision and supervision of local providers.
