On the balance of probabilities the coroner found the fire originated from a fuel leak at the upper portion of the forward right hose connected to burner number 193, causing the balloon fire and resulting multiple injuries and burns; the deaths are recorded as accidental and the coroner issued specific recommendations to the Travel Industry Council and Kuoni regarding risk assessment, passenger information, insurance disclosure and oversight of destination management companies.
