The appeal was dismissed because the weight of medical evidence (x-ray, MRI, CAP reports and independent review) showed established medial compartment osteoarthritis and a complex/degenerative meniscal tear; absence of contemporaneous reporting of an acute knee accident and the imaging features (full thickness chondral loss, meniscal extrusion and complex tear) point to degenerative origin rather than trauma, so ACC correctly declined funding.
