ACC properly revised and revoked cover because contemporaneous medical records contain no evidence of back pain immediately after the accident, medical experts concluded the lumbar pathology was likely degenerative and not accident-related, and ACC's investigation provided sufficient factual basis under s390 to show the original acceptance was in error.
