The ACC167 form in its standard, unaltered form sought a wider authorization (collection, use and disclosure of 'information' for research and evaluation and for the whole claim) than the statutory obligation in s72(1)(c) (authorise ACC to obtain medical and other records relevant to the claim); because completion of the ACC167 as drafted exceeded what the Act required, refusal to sign that broader form was not an unreasonable refusal or failure to comply with the Act and ACC was not entitled under s117(3) to stop the appellant's entitlements.
