The ACC167 in its present, unaltered form sought authority materially broader than the statutory obligation in s72(1)(c); therefore a refusal to sign that form was not an unreasonable failure to comply with a requirement of the Act and did not permit ACC to stop entitlements under s117(3). The respondent's decision to stop entitlements was unlawful and the appeal is allowed.
