On the balance of probabilities the court found the immediate cause of the in-flight fire was a fuel leak at the upper portion of the forward right hose connected to burner number 193 leading to multiple injuries and burns; the deaths of the nine persons were concluded to be deaths by accident; the coroner made targeted recommendations to TIC and Kuoni to address information, risk assessment, selection and supervision of local service providers and insurance clarity.
