On the balance of the medical evidence the condition requiring surgery was wholly or substantially caused by an underlying gradual process (lateral epicondylitis) rather than the single‑event fall; the claimant bore the onus to produce evidence linking the fall to the tear and ACC was not obliged to investigate a work‑related gradual process claim without a formal application and requisite information; therefore ACC's refusal of treatment funding was correct and the appeal is dismissed.
